ACE-031 is a soluble fusion protein consisting of the extracellular domain of the activin receptor type IIB (ActRIIB) linked to the Fc portion of an immunoglobulin, designed as a therapeutic myostatin inhibitor. Originally developed for treating muscle-wasting conditions such as muscular dystrophy, it functions by acting as a decoy receptor that binds to myostatin and other related TGF-beta ligands, thereby preventing them from engaging endogenous cellular receptors that normally limit skeletal muscle growth. By neutralizing these negative regulators, ACE-031 has been shown in preclinical and early clinical studies to promote significant hypertrophy and increases in muscle mass and strength. However, clinical development was halted due to adverse vascular side effects, including minor bleeding events and telangiectasias, which arose from cross-reactivity with other bone morphogenetic proteins (BMPs) essential for vascular homeostasis.
